Application forms can also be downloaded from this website. Completed application forms, copies of academic certificates and identification documents must be submitted together with proof of payment of a non-refundable application fee of USD$10.00 for local undergraduate students, USD$15.00 for local postgraduate students and USD$50.00 for foreign students.
