PLEASE ENSURE THAT YOU HAVE THE FOLLOWING E-DOCUMENTS HANDY PRIOR TO FILLING IN THIS APPLICATION:

Certificates of Secondary and/or Tertiary qualifications. Certificates for the same level obtained at different attempts should be merged into one document

National ID Card or Passport biodata page

Declaration of Employer (for those in employment)

Note: e-documents should be in PDF format. The Maximum size of each PDF document is 1MB.

Step 1: For a NEW USER, click on Register to create your account and fill in the necessary information.

Step 2: Login with your email and password to access the application platform.

Step 3: Fill in the Online Application Form.

Step 4: View/Edit your application. Before submitting your application, ensure that all information provided is correct.

Note: Once you have submitted your application by clicking on the “Submit” button on the “Declaration” page, your application will be locked and you will not be able to edit the Form.

Step 5: Visit the Payment Details page to get information on application fee and modes of payment.

Note: Go to the “Forms” page to download the Cash Deposit Voucher form or the Declaration of Employer form.

Notice to SBMF students (relevant cohorts)

Deadline for submission of dissertations regarding both undergraduate and postgraduate SBMF cohorts whose submission deadline was 14 June has been exceptionally extended to 30 June.
